If you wish to enroll for the summer coverage, please submit your completed enrollment application to the Campus Health Insurance office by the specified deadline date noted below.

Full summer coverage period: 6/1/12 - 8/15/12
Premium: $346.00
Last day to enroll: 6/18/12

If enrolled in the Bridge program: 7/24/12 - 8/15/12
Premium: $104.37
Last day to enroll: 7/24/12

First day of Tucson Campus Medical College classes: 8/1/12 - 8/15/12
Premium: $68.07
Last day to enroll: 8/1/12

First day of Phoenix Campus Medical College classes: 7/23/12 - 8/15/12
Premium: $108.91
Last day to enroll: 7/23/12

You may mail, email, fax or drop off your completed enrollment application to our office.  The Campus Health Insurance office is on the 2nd floor of the Campus Health Service, located between Highland Avenue and the 6th Street Parking Garage.

Benefit information – Please note that once you enroll, you will be automatically re-enrolled and billed in future semesters (each fall and spring) unless you cancel your coverage during the published "Open Enrollment".

There are life events that may qualify you to come onto the Arizona Board of Regents Student Health Insurance Plan at a date different then mentioned above.  For information, please contact the Campus Health Insurance Office at (520) 621-5002.
