Commercial Health Insurance Plans

Currently the Campus Health Service is a participant with most United Healthcare and Aetna plans. If you are a subscriber to any of these commercial plans and would like to obtain information on how to check if we are a participating provider under your plan, please call our Billing and Claims Department at (520) 621-6487.

If you have coverage through a commercial insurance company where we are not a participant under your plan, be sure to request an itemized statement upon checking out after your visit as the Campus Health Service does not file with non contracted insurance carriers. You may also download a Request For Billing Statement (pdf file opens in new window) and send to: Billing & claims Office, Campus Health Service, P.O. Box 210095, Tucson, AZ 85721-0095 so you can file your claim. We recommend that you call your Member Services Department to see what benefits you will have while in Tucson or if you seek care at the Campus Health Services. If you find that your insurance benefits provide limited to no coverage for the care received at the Campus Health Service, you may want to check out either the CampusCare Supplement or the Arizona Board of Regents Student Health Insurance program.

Charges not paid on the day services are rendered will be forwarded to The University of Arizona Student Bursar's account. These charges should be paid as soon as possible to prevent registration holds, cancellation of classes or other inconveniences. You may call The University of Arizona Bursar's Office at (520) 621-3322 for payment due dates. Campus Health accepts cash, check, Visa, Mastercard or American Express.
